Optional Practical Training Current Employer Information Form

All F-1 students on Optional Practical Training or OPT STEM extension are required to update any changes in employment throughout the duration of the dates listed on their EAD card. Students are required to let the Graduate School know as soon as possible regarding any changes in employment.

Please fill out the below form at any point that your employment status changes while on OPT. If you become unemployed, it is important to update our office of this as well.
